MY DIABETES CARE CARD
This card can be used to teach patients about the A1C test, their values and setting goals for their Diabetes ABCs - A1C, Blood Pressure, Cholesterol, Stop Smoking and Self-management Education. It can also be used to encourage other important recommended tests and exams. This card folds up to wallet size.

DIABETES CARE STANDING ORDERS - UPDATED SEPTEMBER, 2020
This tool provides an example of a concept that can be used to optimize the care of patients with diabetes by improving workflow processes through implementation of standing care orders. It is not intended to replace or preclude clinical judgment or more intensive management. Use it as a reminder, to simplify ordering procedures and as a way to continually improve care to adult patients with diabetes. Upon approval from the practicing physician/clinician, standing orders may be initiated by approved office staff. When instituting orders, review information from the patient and his/her chart to apply the protocol appropriately. Diabetes Care Standing Orders may be applied at any patient encounter (does not have to be a diabetes focused visit).

DIABETES CARE TOOL - UPDATED SEPTEMBER, 2020
This tool is based on the American Diabetes Association’s Standards of Medical Care for Patients with Diabetes Mellitus and indicates the minimum services to be provided in the continuing (initial visits have additional components) care of adults with diabetes. It is not intended to replace or preclude clinical judgment or more intensive management when medically indicated. Use it as a reminder for exams or important tests, to simplify record keeping and as a way to continually improve care to all patients with diabetes. A second slightly different version allows for electronic entry and integration in the Electronic Health Record (EHR).

DPP, DSMES OR MNT PRESCRIPTION PAD
Self-management education can be valuable support for patient and clinical practice diabetes prevention and management efforts. When asked why a person chooses to attend DSMES or not, the main reason stated is "my doctor or health care provider told me to go”. This tool can reinforce that message and can have local program information added and be given to patients when they have been referred to the National Diabetes Prevention Program (DPP), Diabetes Self-management Education and Support (DSMES) or Medical Nutrition Therapy (MNT).
